I've been stuck on this for about 5 hours, maybe 6. What I did was make a copy of a site, and put it into another folder (in the same domain). Tried installing with and without install-ex.php method (I've read tens of threads on this forum), tried to access the page(s) as super admin, nothing changes.

As you concluded in your report, this is a JavaScript problem introduced by CloudFlare's Rocket Loader feature. Perhaps I could take a stab at trying to make this play nice with the next Couch release, but no promises...
