I think you have missed registering the cart-modal.php template (which is what should get loaded in the popup).
Please register it (access it via your browser while logged in as super-admin) and let us know if that helped.

The way all Couch templates are registered - access it via your browser while logged in as super-admin. You should be able to see it listed in the sidebar of your admin-panel.
