All codes i input are correct, and when i refresh the browser shows that No templates are being managed by the CMS.

Let us say you want 'about.php' to be managed by Couch.
Assuming that you have already put the two mandatory lines of PHP in the template,
are you sure you took the following next steps properly -
1. Login as super-admin into Couch admin-panel (if not already logged-in as super-admin)
2. Using the same browser in which you logged-in as super-admin above (another tab or another instance of it), visit http://www.yourdomain.com/about.php (substitute http://www.yourdomain.com with your site's address).

Please take the said steps and the template should appear in the admin-panel.
