Is it possible to collect info from a form on the frontend and store it and show it in the couch backend.

What im trying to do is make a user-review section on my site where users can rate a game and write a mini-review. This review would have to be approved before being published and show on the site like a comment, but not the same as comments?

That is what we are working on for the next major release of Couch (along with better user/permiision functionality) - being able to update the backend from frontend user submission.
There is still sometime before we reach there, though.
