How to delete the template created in admin panel?

I already remove the <?php require_once( '../couch/cms.php' ); ?> from template and refresh the page, it does not remove the template from admin panel.

Try removing the physical file from its original location temporarily (or rename it for a little while).

Couch should sense that the original template file is gone and prompt you to remove the cloned-pages (if any exist) and then the template itself from the admin-panel.
